Patient Forms, Map and Directions to Dedham Office
Your first visit to Drs. Lee, Stone, and Cardinale establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We believe that your experience at our office will be different than many of your other dental offices. We individualize your treatment experience by letting you have as much or as little input as you like. We listen and care. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies, and more. There's also background information about our committed team and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
Your First Appointment
To provide a comprehensive picture of your oral health, we recommend a complete oral examination for the first non-emergency appointment. We will begin with a detailed medical and dental history. We will then take all necessary digital radiographs, do a complete periodontal charting to evaluate the current health of your gums, check your occlusion (bite), do a smile analysis, check for cavities, evaluate any TMJ problems, and also do an advanced oral cancer screening with high tech light called VELscope. All this will be used in developing a personalize treatment plan for you.
Our practice is working together to realize a shared vision of uncompromising excellence in dentistry.
To fulfill this mission, we are committed to:
- Listening to those we are privileged to serve.
- Earn the trust and respect of patients, profession and community.
- Exceed your expectations.
- Ensure a creative, challenging and compassionate professional environment.
- Strive for continuous improvement at all levels.
